How or where do I submit specimen to CTA?

Specimens can be submitted by mail via FedEx or UPS with shipping bags and labels provided by CTA. Depending on your location a CTA courier may be available to pick up specimens upon request. All submissions should be addressed to

What do I need to send with my specimen?

When submitting a specimen for testing the following needs to be included: a requisition form including clinical information and morphology / DDX, the patient's demographics (name, DOB, address, & insurance information), as well as any prior pathology reports if applicable.

How do I order supplies?

When you need a new shipment of supplies simply email us at with your clinic's name, address, and the specific material needed and we'll ensure it arrives as soon as possible.

How do I request an amendment or addendum to a pathology report?

If an amendment or addendum needs to be made to a pathology report the best way to do this is to email us at or call our office directly at (503) 906-7300 and ask to speak with one of our Medical Editors. They will be able to assist you with your request and complete it as soon as possible.
